As a remote medical case manager, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. **Responsibilities**
As a Licensed Social Worker on our team, you'll be responsible for:
* Partnering with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs
* Researching available options and resources for members with financial, transportation, safety, and housing needs
* Empowering members to make positive lifestyle and medical choices
* Collaborating with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Utilizing a collaborative process to meet members' needs and improve health outcomes
**Requirements**
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in state of residence
* 2+ years of experience in medical social work or case management
* Masters Degree in Social Work
**Preferred Qualifications**
* Experience working with diverse populations, including children, adults, and seniors
* Knowledge of community resources and services for medical, financial, and behavioral health
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
