Key Responsibilities
As an integral member of our support team, you will be responsible for:

- Resource Connection: Research, identify, and connect families with community resources including housing assistance, healthcare services, educational programs, employment opportunities, and social support networks.
- Advocacy: Advocate on behalf of families and children to ensure they receive the services and support they are entitled to. Navigate complex systems and act as a knowledgeable bridge between families and service providers.
- Assessment and Planning: Assess family needs, strengths, and goals. Develop individualized support plans that address unique circumstances and promote long-term stability.
- Case Management: Maintain detailed case files, document progress, and track outcomes. Ensure accurate record-keeping and compliance with organizational and regulatory standards.

Qualifications and Requirements
