Our GA Community Action Treatment (CAT) program is recruiting for a Therapeutic Mentor. Program is located in Region 1 and 2 (near Cherokee county and surrounding areas).
Georgia Community Action Treatment (CAT) is intended to be a safe and effective alternative for children with serious behavioral health conditions that are creating crisis in the family. Upon successful completion, the family should have the skills and support system needed to maintain improvements made during services. The goals of the CAT program are to:
Strengthen the family and support systems for youth and your adults to assist them to live successfully in the community;
Decrease out – of – home placements, including psychiatric hospitalizations and Department of Juvenile Justice (DJJ) involvement;
Improve family and youth functioning;
Decrease substance use and abuse;
Transition into age - appropriate services; and
Increase health and wellness.

RESPONSIBILITES:
Comply with all standards to ensure the health, safety and respect of consumers we serve
Act as a member of the CAT team as scheduled and needed
Provide crisis follow-up services within 24 hours of crisis disposition
Complete face-to-face follow-up with individuals, as assigned, after crisis response
Provide one-on-one sessions to youth and their family to address issues related to daily living, social skills, and appropriate communication techniques
Complete documentation as required by Benchmark Human Services and state and federal regulations
Assist in the completion of assessments, treatment/care plans, safety plans, and crisis debriefing with the youth and family
Assist the individual in identifying resources for transition of services and long-term recovery
Attend all scheduled training and staff meetings
Assist with crisis response as required, including face-to-face response within an average of 60 minutes
Complete all necessary documentation in accordance with applicable policies and procedures
Report any suspected abuse, neglect or exploitation to supervisor or department head
Facilitate communication among the CAT team, youth, family and other service agencies involved regarding home, academic, and/or vocational related needs
Comply with all standards to assure the health and safety of all individuals
Other duties as assigned
QUALIFICATIONS:
Minimum Qualifications:
Must have high school diploma or GED
Experience working with youth up to age 21 years old with mental illness, emotional disorders, and substance-related disorders who are experiencing emotional or behavioral crisis
Valid driver’s license.
Preferred Qualifications:
Two years’ experience working with youth diagnosed with behavioral health disorders, substance use disorders and/or intellectual/developmental disorders
Knowledge of community resources and supports for youth and families
